Application Forms

First, we need to talk. Later, we may ask you to submit two forms.

If we ask you to complete our forms, please follow these instructions.

1. There are two forms which must be completed to apply for a loan:
   • Loan Application Form
   • Personal Financial Statement
2. Download both forms and save them to your computer. You can type your information directly into the forms or fill them in by hand. If you have a current Personal Financial Statement of your own, you may submit that in place of our Personal Financial Statement form.
3. A non-refundable fee of $200 is required with the forms.

Print and mail both forms along with a check for $200 made payable to “The Progress Fund” to:
